The Children's School of Music - Registration Form

Please complete one registration form for each child/class attending.

Fall and Winter classes are offered by session. A $55 down payment is required before classes begin. Pre-payment for each of the Fall and Winter sessions is required when classes begin.

For our special Summer Camps, full payment is due in advance and at time of sign up.

Submitting your registration makes us aware of your interest in a specific class. If there is not room in the class, we will contact you immediately. Please note that we cannot firmly hold a position in your desired class until we receive your downpayment.

Please note that we require 1 form per child, but if your child is taking two classes, such as Guitar and Piano, you may use a single form and indicate both classes in the box at left.
